Strangulated obstruction(with torsion) of small bowel with no necrosis












Because ascending colon (Ac) is collapsed in Fig.25-Fig.27 of the last line, distended small bowel indicates mechanical obstruction. Most of distended small bowel are gasless, ascites in pelvic cavity (Fig.28 and Fig.29: reference mark) and mesenteric stranding (black arrowhead) in Fig.13-Fig.16 strongly suggest strangulated obstruction. A of Fig.5 occludes at O of Fig.19. Although 1 of Fig.5 progresses to 81 of Fig.17, presence of collapsed small bowel (SB) in Fig.20 and Fig.21 suggests that black arrow of 79 in Fig.19 is a true site of the obstruction, indicating closed loop formation. Circle number 7 of Fig.27 from circle number 1 of Fig.21 are the simple obstruction of cephalad side. Slightly diminished contrast-enhancement of wall of closed loop represents equivocal viability. Surgery revealed ischemic (but viable) small bowel of 150cm with edema and redness (Fig.A: between white arrowheads) by adhesion and 180 degrees torsion.
