CT of Acute Abdomen

Epigastric Pain series 2 EXPERT COURSE Answer [EE Case 7]

Gastric anisakiasis

CT images demonstrate edematous wall thickening (black arrowheads) of the whole stomach without confining in antrum. Endoscopy showed an anisakid insect body in body of stomach (Fig.A: black arrows). The symptom was remarkably improved after extirpation of insect body. Anisakiasis should be taken into consideration if CT images show extensive submucosal edema without confining in antrum of stomach.

Reference case (gastric anisakiasis): A 61-year-old female presented with severe epigastric pain that suddenly occurred after having eaten sushi (Japanese raw fish) 12 hours ago. Temperature: 36.7 degrees Celsius. There is tenderness in epigastrium. CT images show extensive wall thickening (black arrowheads) by submucosal edema just like the above case. Endoscopy revealed an anisakid insect body which was sticking fast to mucosa of body of stomach (Fig.B: black arrow).
