図１１の1〜図７の8までの直腸は右側へ圧排され，図８の9からＳ状結腸となる．図７と図６の▲の部分はＳ状結腸の，約4cmの壁欠損部（discontinuity）で，そこから腸管外へ露出している糞便が描出されており（△），Ｓ状結腸穿孔の診断は容易である．憩室や腫瘍性病変は認めず，Ｓ状結腸壁は普通に造影され原因疾患はＣＴでは不明である．12時間後腹痛が増強し，腹膜刺激症状を呈したので手術となった．混濁した腹水が多量にあり，Ｓ状結腸部分壊死と500円玉大の穿孔を認めた（図A：↑）．Ｓ状結腸切除とHartmann手術を行った．病理：perforation of sigmoid colon and peritonitis，ischemicな変化の可能性も否定し得ない．

文献考察：左側大腸穿孔による腹膜炎症例で30%以上の症例で（Hartmannでなく）切除＋primary anastomosisが可能（１〜９）。徳洲会グループの左側大腸穿孔による腹膜炎症例の手術は99%Hartmann手術である。もっと積極的にprimary anastomosisを行ってもいいのではないか.
